I lost my health insurance card and I'm worried someone might misuse it. What should I do?

Notify the police immediately if it was stolen. Take precautions to store your card in a secure place; there is currently no way to prevent use of a lost health insurance card. Submit an Application for Reissue of Health Insurance Card (loss / damage) to have your card reissued to the return destination.
