If my child (dependent family member) has got a job at a company, what processes should I take?

MSDKK and MSDGK employee: Please apply for the transfer of family members in the e-pay application portal site. Following the submission of the application form, you will receive the form called "Notification of Health Insurance Dependent (change)(deduction)".  Complete this form and submit it with your documents to Payroll Inc.

MSD AHKK employee: Please contact to i6 Consulting Sharoshi Hojin in charge of AH. You will receive another form called "Notification of Health Insurance Dependent (change)(deduction)". Complete this form and submit it with your documents to i6 Consulting Sharoshi Hojin in charge of AH.

ORGANON KK employee: Please contact to i6 Consulting Sharoshi Hojin in charge of ORGANON. You will receive a form called "Notification of Health Insurance Dependent (change)(deduction)".  Complete this form and submit it with your documents to i6 Consulting Sharoshi Hojin in charge of ORGANON
