I was told by the hospital to get the Maximum Ceiling Amount Applicable Certificate. How can I apply?

If you are using a Myna Health Insurance Card, the limit information will be provided without consent, and you will not need a certificate of application of the maximum amount, so please use the Myna Insurance Card.
If you do not have your Myna Health Insurance Card, Please download the application form from the website and submit it to Value HR Health Insurance BPO Center, in charge of MSD by post mail.
