When my child received a MR vaccination and hepatitis B vaccination, the receipt was issued by totaling the two vaccination expenses, and the receipt did not specify the names of the vaccination. How can I apply for a subsidy?

Please fill in each application form with the required information and attach only the original receipt. Also, attach a copy of the page of the maternity passbook or detailed statement which specifies details of the vaccination.
