I lost my health insurance card etc. and I'm worried someone might misuse it. What should I do?

Notify the police immediately if it was stolen. Take precautions to store your card etc.(Myna Health Insurance Card, Eligibility Verification Certificate etc.) iin a secure place; there is currently no way to prevent use of a lost health insurance card etc.. Submit an Form for Loss of Health Insurance Card etc.(Notice of loss / damage) to the return destination.

 

Please use your Myna Health Insurance Card From now on.  
Those who have not been issued a My Number Card, or those who have not registered your My Number Card for the use of the Myna Health Insurance Card etc.,  "Eligibility Verification Certificate" will be (re)issued, so please submit the "Eligibility Verification Certificate Loss/Reissue Application Form" to the  return destination.
