Please note that once you join the voluntary continuous insured person, you cannot withdraw from the program except in the following cases:
(1) Two years have passed since the date you became an insured person.
(2) Upon your death.
(3) You do not pay your insurance premiums by the date due.
(4) You begin employment and become an insured person under another health insurance or similar program.
(5) You join the Medical Care System for the Advanced Elderly as an insured person, etc.
(6) If you applied to have your status as a Voluntarily and Continuously Insured Person cleared, the last day of the month including the date on which the application was received
If you forfeit your eligibility as a voluntarily and continuously insured person during the coverage period because you start another job and join another health insurance plan, contact Value HR, Health Insurance BPO Center, in charge of MSD via e-mail or by telephone, complete the "Application Form for Loss as Voluntarily and Continuously Insured Person" by returning your MSD health insurance card(until December 1, 2025) ・ Eligibility Verification Certificate(including your dependents )with a copy of the "Eligibility Verification Certificate" or "Notice of Eligibility Information" from your new insurer attached. Return your health insurance card (until December 1, 2025) ・Eligibility Verification Certificate promptly if you forfeit your eligibility for other reasons as well to Value HR, Health Insurance BPO Center, in charge of MSD.
If you use your health insurance card etc. to receive care at a medical care institution after forfeiting your eligibility as an insured person, you will be billed for the medical care costs (i.e., the portion paid by health insurance).
* If you lose your eligibility for cases 2.4.6., please download the "Application Form for Loss as Voluntarily and Continuously Insured Person" and send it to Value HR, Health Insurance BPO Center, in charge of MSD by post mail.
