| Notes: | If your health insurance card is lost or stolen, please notify the nearest police station first to prevent it from being misused and write down on the application form the receipt number issued by the police. If you later find your health insurance card, please be sure to return your old health insurance card to the MSD Health Insurance Society. *If an insured loses his/her health insurance card due to carelessness and the lost health insurance card is used by a third person, the medical expenses incurred by such third person will be charged fully to the insured person himself/herself. |
