Apply to the Health Insurance Society if the name of the insured person or a dependent has changed or you want to add a new family member as a dependent due to marriage or other reason.
If you are an MSDKK or MSDGK employee, change your name by the “workday®” portal site for the insured person. Change your dependents name by logging in to the e-pay application portal site, click the “Add or Remove Eligible Family Members tab” and select the applicable item, and apply for a “Notification of Change in the Number of Eligible Family Members”. After electronic filing is completed, you will receive a set of papers in the post mail from Payroll Inc. Please fill in those papers and send them back to Payroll Inc. in the enclosed return envelope.
For the name change of the insured person : Health Insurance Card(until December 1, 2025) or Eligibility Verification Certificate (for the insured person and all dependents)
For the name change of the dependents:Health Insurance Card(until December 1, 2025) or Eligibility Verification Certificate (for the dependents of name change)
